To register, contact the National Children’s Leukemia Foundation (info@leukemiafoundation.org, or 1-800-448-3467) and ask for a registration packet.  The packet includes a consent form for you to sign and a medical consent form to be signed by your physician.  Complete the forms and email or fax them back to us.  We will call to confirm your enrollment and give you the dates/times of class.
